API has no Bedrock invocation or SQS send permission.
Worker owns relay, consumption, tools, inference and result persistence; not approval.
Case creation requires a different, independently scoped reviewer and transactional rechecks.

Cognito, CloudWatch, Secrets Manager, S3 and ECR are supporting services omitted from this request/worker slice.
